Any person with a
disability who resides in the agency's service area may request Case
Management services. For individuals and families served in the
Early Intervention program, Case Management services are available
through Child & Family Connection.
Case Management
helps fulfill the mission of WAC by:
Facilitated
participant choice in the creation of individual program plans.
Monitoring the
delivery of services for quality, coordination and cooperation between
programs.
Advocating on behalf
of participants, both within and outside the agency.
Assisting
individuals to develop realistic and worthwhile goals.
